Transition Youth Programme
The Transition Youth Programme (TYP) is an education programme for children and young adults from 13 of age and upwards with kidney disease.
TYP is designed to equip children and young adults with the knowledge, tools, and confidence to ease their way into adult services independently.
TYP covers the following modules, each module is delivered during an evening workshops which includes food, and is followed by a social activity:
- The future is bright.
- You can do it! Kidney disease does not need to define you, you can lead the life you want. Time to believe in yourself!
- Learn from other young people, how they transitioned and how they have followed their dreams.
- This will help build confidence, resilience and help you set your own goals and know how to achieve them.
- Wellbeing
- Emotional wellbeing effects everyone, it is OK to not be OK.
- Learn from experts on how to create your own emotional wellbeing toolbox.
- This session will provide you with some tools and tips on how to take control of your own emotional wellbeing.
- Taking Control of CKD
- Taking your health into your own hands, managing your appointments around your life, and attending clinics on your own,
- Learn about your Health Rights, importance of having a purpose, managing your own health.
- This session will help you know you can attend appointments on your own and managing your own treatments and record your progress.
- Healthy Romantic Relationships
- How to build safe, healthy relationships for the future you want including starting a family.
- Learn about positive and negative relationships, sexual health, fertility options.
- This session will help you set your own boundaries for the relationships and future you want.
- Treatment options and life impact
- Deciding on the treatment option that suits your lifestyle.
- In this session you gain basic knowledge of all treatment options
- You will be able to make an informed decision about the best treatment option for you!
- Diet and the kidneys
- Learning what foods can help you and your kidneys without ruining your social life!
- Dieticians will provide information, advice, and tips!
- Good out for food and socialising does not need to be restrictive!
- Dialysis
- The pros and cons of each haemodialysis treatment options.
- Learn more about these treatment options.
- This session will provide full details of all haemodialysis treatment options.
- Transplant
- What does a transplant mean for you and what are the options?
- Learn for transplant specialists about the options available.
- You will be able to make an informed decision about this treatment option.
Calling All Parents and Carers!
We’re excited to invite you to a Parent and Carer Support Session, running alongside the TYP.
This session is a great opportunity to share your experiences, connect with others, and receive support in a welcoming environment. One of our peer mentors will be present to guide the conversation and offer insight.
Everyone is welcome—whether or not your child attends the TYP.

Activity Days
Activities are adapted for different ages and abilities and led by trained staff who make sure everyone feels welcome and supported so you can focus on having a great time! There may also be opportunities for short residential trips for some age groups during the year subject to interest and suitability.

Easter and Christmas Day Out
In previous years our family days have been a huge success – parents and children told us how much they cherished the opportunity to relax and have fun altogether. Building on that we’re excited to make these seasonal events a regular part of what we offer, giving families the chance to share joy and feel supported in a warm welcoming community.

Counselling
Feeling overwhelmed by the challenges of managing an illness or the pressure of more independence can be daunting. We can offer empowering and supportive counselling and therapy solutions tailored for you. With our engaging one-to-one sessions, you can build up the confidence you need. We understand the importance of mental health at every stage of development, and want to enable an easier path to independence and a brighter future.

Youth Volunteering
About the Opportunity
We are currently looking for 10 Young people aged 14-18 to join our volunteering opportunity, designed for Children and Youth with kidney disease/lived experience to help create and run a project that supports other young people affected by kidney disease.
This will be a youth-led volunteering opportunity, which means you and your team will help decide what project to create – with support from our staff team.
Together you might:
- Create an awareness campaign
- Plan a youth event
- Design resources for other young people with kidney conditions
- Run fundraising activities
- Share experiences to help others feel less alone
You don’t need any previous experience – just an interest in helping others and working with a team.
Why this matters:
Young people living with kidney disease often face challenges that others don’t see.
You will help:
- Raise awareness
- Support other young people
- Improve experiences for future patients
- Make sure youth voices are heard

Programme Timeline
4th December
- Welcome and introduction session
- Meet the group and learn more about the programme
January to March
- The Youth Volunteer Programme will begin in January and run for three months
- Participants will take part in:
- Weekly online meetings
- One in-person meeting each month

Parent and Carers
Caring for a child with kidney disease can feel overwhelming at times, but you don’t have to face it alone. At Popham Kidney Support, we’re here to support parents and carers with practical help, emotional support, and opportunities to connect with others who truly understand the journey.
Our Parents & Carers community provides a safe, welcoming space where you can share experiences, ask questions, and find encouragement from people who have first-hand experience of supporting a child with kidney disease.
We offer:
- Parent & Carer Support Groups – Meet other families, share experiences, and build lasting connections in a supportive environment.
- Parent & Carer Peer Mentors – Connect with trained parent and carer volunteers who have lived experience of supporting a child with kidney disease. They offer understanding, reassurance, and a listening ear from someone who truly understands.
- Grants & Financial Support – We offer grants to help ease some of the financial pressures that can come with caring for a child with kidney disease.
- Information & Signposting – Helping you access the right support, services, and resources whenever you need them.
Whether your child has recently been diagnosed, is receiving treatment, has had a transplant, or is living with long-term kidney disease, we’re here to support you and your family every step of the way.
